Connecticut Institute for Communities, Inc. Connecticut Institute For Communities, Inc. (CIFC) / CIFC Health seeks to hire a full-time Peer Recovery Coach to work in our Behavioral Health Department. Under the direction of the Lead BH Community Health Worker, this position is responsible for providing support and guidance to individuals seeking substance use disorder treatment. The peer recovery coach, an individual in sustained recovery, can utilize their experiences to mentor or coach, provide and advocate effective recovery‑based services. Peer mentoring is inclusive of information and guidance relevant to a client’s recovery needs (social skills, substance use recovery supports, training, mentoring, behavioral skills management, employment, and recreational activities). The peer recovery coach will assist the team in supporting client goals for recovery and with interventions that will assist clients navigate their journey of recovery. This position is responsible for providing supportive social services including bi‑directional referrals and follow‑up as required in accordance with CIFC Health standard of care, assisting with financial insurance assistance to CIFC Health patients seeking Behavioral Health/Substance Use Disorder treatments and their families, and supporting CIFC Health providers by providing assistance with substance abuse treatment support and referrals. Essential Job Responsibilities: Provide education on substance use disorder and skills training in illness and recovery management to clients and their families. Work collaboratively with clients to assess their hopes, strengths, accomplishments, and challenges regarding their recovery goals. Collaborate with other care team members to develop and assess needs and create goals and treatment plans inclusive of the clients’ personal interests, strengths and challenges regarding recovery. Actively identify and support referral and linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self‑help groups, professional services, etc.) that support the clients’ recovery goals and interests. Provide in‑person and telephonic support, coaching and mentoring to help clients meet their recovery goals. Provide informal counselling, discuss issues and concerns related to basic needs and community resources, and facilitate peer recovery groups. Facilitate healthcare and social service system navigation. Screen for social determinants of health. Determine eligibility and enroll individuals into health insurance plans. Serve as point person receiving and facilitating referrals for Substance Use Disorder treatment services in collaboration with providers. This includes internal referrals from the CIFC Health department and community referrals to withdrawal management and inpatient substance abuse/co‑occurring treatment programs. Collect clinical data and records prior to intake appointment, including records from other facilities, patient‑completed ratings, and appropriate collateral. Communicate with other providers internally and externally regarding substance use disorder referral outcomes by creating provider letters, discharge letters, and summaries in cooperation with providers. Participate in clinical team meetings and communicate with providers involved in patients’ care to facilitate all communication and coordination with the team. Attend provider team meetings to promote collaboration of service and treatment providers. Attend and participate in individual and/or peer supervision as directed. Adhere to all HIPAA regulations, including those related to the heightened protection of health records, and maintain confidentiality at all times. Fulfill all compliance and training responsibilities related to the position, including compliance with and enforcement of CIFC and CIFC Health policies and procedures. Perform other related duties as assigned. Requirements: Successful candidates must possess a minimum High School Diploma or equivalent. (Associate/Bachelor’s Degree preferred). Lived experience and/or in recovery from substance use for a minimum of 2 years. Certified Addictions Recovery Coach preferred or willingness to obtain; willingness to obtain Certified Addictions Recovery Coach status. Community health experience and/or community resource knowledge strongly preferred. Computer and electronic health record experience, including using the internet. Bilingual abilities preferred.
